Development Roadmap

Six phases from foundation to NHI capitation at scale. 24 months to first revenue. 54 months to EBITDA breakeven. A structured, milestone-driven path to national healthcare impact.

The Path to First Revenue

The SAHPRA clinical validation sequence is the critical path. Everything else — scheme partnerships, physician onboarding, infrastructure build — runs in parallel and feeds into this gate.

🔍

Month 0–6

LLM selection & fine-tuning on SA corpus

📋

Month 6–12

Retrospective validation study

🏥

Month 12–18

Prospective pilot — 3 sites, ≥1,500 consultations

💵

Month 19–24

SAHPRA file complete → First revenue

Quality gates for SAHPRA: AI sensitivity ≥92% on red-flag conditions, specificity ≥88% on EML-compliant prescribing, physician override rate <15% across all disease categories, bias parity within 5% across demographic groups.

CEO not in Phase 0: The initial team is technical and clinical. A CEO with scheme partnership and healthcare commercial experience joins in Phase 1 as part of the seed-funded leadership expansion.

Seven Parallel Workstreams

The technical build runs across seven workstreams in Phase 1, all converging at the Phase 2 prospective pilot gate.

🔒

1. Data Platform

FHIR R4 data store, consent ledger, audit log, SA ID integration, POPIA-compliant data residency on AWS Cape Town.

🤖

2. Clinical AI

LLM fine-tuning on SA corpus, deterministic rules engine (EML/ICD-10/STG), red-flag escalation logic, explainability chains.

👔

3. Physician Workflow

RabbitMQ case queue, physician review dashboard, approve/modify/escalate UI, HPCSA verification integration, audit trail.

📱

4. Patient Interfaces

React 19+ PWA (offline-first), USSD gateway (Africa's Talking), audio mode, multilingual clinical flow (11 languages).

🏦

5. Integration Services

Healthbridge claims API, pharmacy e-prescription network (MediFin/ScriptWise), NHI readiness FHIR API.

📈

6. ML Operations
